A complaint is submitted through the portal. This can be done anonymously if the reporter wishes.
Administrators receive an email notification and add the incident to the relevant register.
The incident is reviewed by the relevant administrator/s and informs relevant stakeholders.
Administrators follow-up the report and work to find a resolution.
A risk assessment is carried out to avoid the incident occurring again.
Administrators update the register and they continue to monitor the incident and any related risks.
